It is estimated that the cost of related hospital readmissions are around $18 billion per year for Medicare beneficiaries.  Unplanned hospital readmissions are typically caused by hospital-acquired infections, premature discharge, lack of adequate discharge planning and coordination, poor communication and patient education, and prescription drug complications.  Hospital readmissions are bad for the patient and seen as poor quality care by some.  It has been my experience as a case manager that patients tend to believe they were discharged from the hospital too soon is what brought them back.  The Center for Medicare and Medicaid Service is now monetarily penalizing hospital for excessive 30 days readmission for the same diagnosis.  There are tools out there if implemented correctly to reduce hospital readmission, but I would like to see more research done on how to prevent hospital readmission for the disenfranchised and disadvantage population who are often times readmitted due to poverty, lack of education, lack of caregivers/support, community resources and the ability make follow-up appointment.
